# Evidence and assumption register

Version: 2026.08.31-applied-1

Use: Log one claim per row, the evidence that would change the decision, and the next test. The included example rows use the fictional MesaLift case.

Important: The MesaLift entries are fictional examples, not client results or product specifications. Replace them with evidence from your own controlled work. This planning tool is not legal, financial, engineering, compliance, safety, or investment advice.

## Instructions

1. Duplicate the template and give every assumption a stable ID.
2. Record direct evidence separately from interpretation; never invent a quote or sample size.
3. Before the gate review, assign each assumption a current status and an owner for the next test.
